Intermediate care is a term to describe a range of services that provide short-term support for older people, avoid unnecessary admission to hospital or long term care and to help discharges from hospital.
We provide Intermediate Care at our Morton Court Care Home, and support our local Primary Care Trust to provide a service at Olsen Court, our Housing with Extra Care scheme.
Short Term Care is 24 hour Personal Care provided for older people within our care homes following ill health or to enable their family and/or carers to have a break.
